Fife Council want to find out how services and local groups can help people with learning disabilities lead good lives.

One of the things they are really keen to find out is about the help and support people might need to do more things, make new friends and develop romantic relationships.

They want to know what you think are the kind of things they could help provide.

Fife Council have asked Heartfelt to help with this. This survey is just some questions to help you have your say.

If you are an individual who gets support, you may want to ask someone to help you with this. This could be your parent/carer, friend, family member, support worker etc.

Click here to download their questionnaire or click here to complete the online version.

Paper copies of the questionnaire should be sent to:

    David Douglas,

    Heartfelt Limited,

    2 Bruce Street,

    Dumbarton,

    G82 1HX.

You can submit a response to this questionnaire up until Tuesday 28 February 2017.
