Survey to evaluate transitions for 14-25 year olds with ASN
Created: 03/11/2016It has been four years since the introduction of the Charter for Transition was launched in Fife. Since then new legislation has been developed and social work adult and children services in Fife have integrated into new partnership arrangements with Health and Education.
Fife Health and Social Care Partnership and Fife Education and Children’s Services are now ready to evaluate the effectiveness of the tools they use to support transitions. In order to gain a comprehensive understanding of the experiences of young people and their families as they go through transition, this review will include consultation with young people, families and practitioners involved in the transitions process.
This review will highlight examples of good practice and identify potential areas for improvement and recommendations for updating the available transitions resources in line with recent legislation changes. ARC Scotland is supporting the Fife Health and Social Care Partnership and Fife Education and Children’s Services to undertake this review and have developed a survey for practitioners across all sectors who are in some way involved in supporting transitions for young people with additional support needs aged 14 - 25 years in Fife.
